Mayuri and Aizen ,one being pathologic, crazy arogant/obsesive control freak, and the other has the ultimate god complex so far in the series.It's safe to say those two are mental, now for somebody else:
Byakuya is prety bad whit his "I'm better than you no questions asked" mentality ,snobish atitude,and condescending way of speaking, some of it is because of upbringing but still..
Ichigo, has been cocky many time he shouln't have been (maybe he knows he's technically imortal lol).
Kenpachi is a bit overconfident but its not like he'l regret it if it ends up killing him.
Do dead guiz count? Brangar was prety bad, and it did kill him in the end.
Szayel, Noitora, Grimmjow was/is prety cocky too but he didn't fail because of that.
Oh Yama-ji might be , aldough he is all that, he did downplay Ukitake & Shunsui in theyr convo a bit too much even while stateing theyr high level of skill.

Aizen : the guy who wants to be God. nothing to add.
Byakuya : that guy knows two words, "law" and "pride".
Ikkaku : if he dies because he doesn't want to use bankai, that is his problem but if he endangers his mission and other people, no.
Yumichika : same as Ikkaku.
Barragan : self-proclaimed king of HM, still considers himself the strongest despite being #2 (at least he has an incredibly powerful ability), sit on a throne while his underlings fight and die
Ichigo : his speech about how he wanted to defeat Ulquiorra was stupid. He could never have won. I like the fact that he wanted the fight to be fair but let's be honest, Ulquiorra was by far superior to Ichigo (not taking Ichigo's new form into account). Bankai Ichigo with mask was more or less equal to unreleased Ulquiorra and Ulquiorra could still release twice. And his "I will defeat you" when facing released Yammy, quite cocky for someone who doesn't even have half his reiatsu. And now he wants to defeat Aizen. Good luck.

If he would do that, that would mean ichigo fights because he wants to fight but as far as we know ichigo fights only because he has to fight. And when you fight only because you dont have any other options you dont start to play around.
