Like all other shinigami, Hanatarō possesses a zanpakutō, even though he is rarely seen carrying it. Since his skills lean more towards medical support and not battle, Hanatarō tends to misplace his zanpakutō as he rarely needs to use it. Hanatarō's zanpakutō is named Hisagomaru (瓠丸, lit. gourd of purity), which is revealed in the Bleach Character Book.[4] Hisagomaru's shikai command is "fulfill" (満たせ, mitase?).[5]
Prior to its shikai, Hisagomaru has a red gauge on its blade. Hisagomaru is strange in that it heals wounds instead of causing them. While this would seem to make attacking fairly pointless, doing so fills the gauge on the blade. The severity of the wound determines how much is filled. When the gauge is full, Hisagomaru automatically initiates its shikai and transforms into Akeiro Hisagomaru (朱色瓠丸, lit. crimson-colored gourd). In this form, it takes the shape of a scalpel and concentrates every wound it has absorbed into one massive attack. Subsequent attacks also have the healing restriction lifted, allowing Hanatarō to attack normally after the transformation, though Akeiro Hisagomaru's form makes that difficult.
I gave it an 8, I love how unique it is, too bad it's never been used in the manga.

i give it an 8 too. it did damage a menos grande as fillery as it might have been still good to see it in action....plus its funny that of all ppl he has a blood-based zanpanktou like the obvoiusly uber-powerful Urahara.:D but his is almost cooler because he doesn't need his own blood

I gave it a 6. It's useful for defeating an opponent in one shot, and for healing one's comrades in the midst of battle, but it also had many downsides.

Ages ago, I've thought of a similar ability (but usable at will, not only when filled) for Unohana zanpakuto. Nice to see Hanatarou with this (even if, not so powerful).

I gave it an 8 because whenever Hanataro wants to make a peanute butter and jelly sandwich all he has to do is whip the little guy out and smear his jelly on them buns. LOL!!
Also his zanpakutou confirms the theory of "It's not the size that counts, it's what you do with it." HA!

The idea is fresh, but it's such a double-edged sword that requires the wielder to have crazy persistence like Ikkaku or ninja stealth like Soi Fong, IMO. Unless Hanatarou is a secret Hakuda badass, he should try his best to stay away from the front lines. 6/10 from me.

It''s unusual. It made for a fun omake. It really fits both Hanatarou and the 4th division (everyone underestimates the healers until they turn their knowledge against you XD) and it must be great for dealing with difficult patients. But for dealing with hollows, not so much.
So all in all, 7/10

Furato; Didn't you know? Hanatarou is Aizen's inside man. Why else help Ichigo? For Rukia? Noone likes Rukia in SS except Byakuya and Renji. And her captain, but he's a nice guy, so...
Tactically, partner him with ikkaku and let Ikkaku get beat around like he so loves with Hanatarou behind him. I mean, just give him a partner and it's too good.

Agmaster: Too bad Ikkaku doesn't seem the type to tagteam, half the fun's in the bleeding. But I suppose Ichigo'd appreciate the ability to insta-heal, so that he can fight on and on like the Energizer Chappy. Renji and Rukia should've brought Hanatarou along on their Spanish desert date. Hisagomaru looks like it works faster than Shun Shun Rikka (or maybe Orihime's been dealing with worse injuries so far).

Actually, I think Ikkaku can appreciate actual in battle tactics enough to tag team. He might be averse to not feeling the damage at first, but hey this way he gets to fight longer and may even pull out a tag move with Hanatarou.
Ichigo is too....shounen protagonist to have in battle heals. It's like a combination of too tough guy/retarded to ever think of or properly use such a support. He always gets healed post battle. Of course, this is also so the healer won't get hurt, but hey...Hanatarou can heal himself, right?

its a butter knife. I shall take a picture of it and upload it. THEN I shall show it to you >=D bohahahahahahahhaa. But anyway, I think its awesome kitchen knife, scalpel or w/e. Its got awesome strength. Its the perfect zanpaku-tô to carry out the message that size isnt everything xDDD.

Unless you're Akabane from GetBackers, they're a little useless for attacking. xp
